What Do I Need to Bring to My Consultation?
To expedite your first appointment, please have your Patient Information Packet completed prior to your appointment and send it in by mail, fax, or email. If you’re a new patient, please fill out the New Patient Packet and bring it to your appointment, as well.
We’ll send both forms to your email once you’ve made your initial consultation, or you can download both forms here.
What Can I Expect During My Consultation?
Your appointment will include an examination to diagnose your pain and offer the best pain management solution. During this time, you’ll be able to ask any and all questions regarding your pain condition and possible treatment options.
How Can I Prepare For My Pain Consultation at CSC?
Although optional, you can prepare for your appointment by making a chronological list of your medical history as well as a list of questions to ask one of our pain doctors. You can also bring a pain diary (if you have one) or any photos or videos that show the progression of your symptoms.
