Please remember to be seen by the doctor you must bring your imaging studies on either computer disc or the actual films. Xray, MRI or CT reports is not enough we need the actual films. You must also bring with you your previous medical and surgical records. After checking in with the receptionist, you will be escorted to one of the exam rooms. Dr. Farley or Dr. George will review your medical history, current problem and imaging studies. At the completion of the examination you will have time to ask all your questions. At the end of the visit you will understand what is causing your pain, how we are going to fix your pain, and you will know how long you will hurt.
Dr.Farley and Dr. George do not provide narcotic medication prescriptions unless you have recently undergone surgery or are scheduled for surgery in the near future. All other narcotic prescriptions must be filled by your primary care physician. Any prescriptions and authorizations for renewal that have been given to you by one of our physicians should be requested during regular office hours when your records are available.
All patients are encouraged to call with any questions they have regarding a medical problem or prescriptions. For answers, you may call our office at (214)782-9222. It is important to note that these are for routine questions and not emergencies. Results for tests that have been performed should be available approximately one to two weeks after the test. For this reason, please wait at least one week before calling the office for results.
BEFORE YOU LEAVE
Dr. Farley and Dr. George will make every effort to carefully diagnose your problems and treat you with appropriate interventions. It is very important when you leave our office, that we have answered all your important questions regarding your illness. At the end of the visit you will understand what is causing your pain, how we are going to fix your pain, and you will know how long you will hurt.
Although health insurance is designed to help you meet the cost of medical services, it seldom covers the entire cost. The primary responsibility for payment, therefore, rests with you. Your policy is a contract between you and your insurance company, not between your doctor and the insurance company. We will provide you with the necessary information and will assist you whenever possible. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, co-insurance or other balance not paid by your insurance company.
We accept most group health plans and are happy to file claims with your insurance company as a courtesy. Important note: Please remember to bring your insurance card and a photo ID with you to each appointment. If you don’t bring your insurance card, we will request payment in full for your visit. If you forget your photo ID, you may have to reschedule your appointment. Federal laws against identity theft now require us to verify our patients’ identity.
